The National Environmental Compliance Handbook (190-VI-NECH-First Edition, October 2003) has an updated version of Environmental Evaluation Worksheet, Form NRCS-CPA-52 dated October 2003.

Find below the updated Montana form and instructions. More detailed instructions for completing Part J of this form can be found in the National Environmental Compliance Handbook, Section 610.71. View the documents on-line, but save them to your computer before entering information.

Part N of this form can be signed by anyone including a Technical Service Provider (TSP). Part P and Part Q must be completed by an NRCS employee. Part R must be signed by the district conservationist.
